Make sure the form is filled out completely. If the school office statement is signed by the counselor, dram and/or register-no grades are needed to be verify. This signed portion stated the driver has met all the requirements.

1.enter the eff date 2. Change the rebate to a Y(of you want the discount to go on now or if the renewal has already been billed) 3. Leave it as an N. (If you want the discount to go out in at the renewal and it has not billed) 4. Of any type of TDE entry, enter Y in the match TDE. This will match the GSD entry with that entry provided the eff dates are the same

If the recertification proof or notification of GS type change is received for a policy & it is after the 27 pend how would you process?
User should make the GS premium adjustment Entry from the CDH screen effective the date SF received proof of it.
Student name, institution name, copy of students transcripts showing the completed degree with a cumulative grade point average of at least 3.0 or B letter grade average. & proof of graduation and/or qualifying grades must be submitted for each qualifying driver
