Carefully monitor the goals set to help clients keep them realistic. C. Work on goal setting, even if it is the whole discussion that session, to make sure they can accomplish every goal.

B. Carefully monitor the goals set to help clients keep them realistic. OR I can see why "C" might be a reasonable option as well. I will explain my thinking and also acknowledge it just might be a bad question. I was thinking a "coach approach" would be more gradual and let the clients discover over time by learning through their own process of goal setting to gradual introduce and design goals for themselves that they succeed at more often. It is considered rare in coaching to spend an entire session on goal setting. This is often done in the health education approach but not as common in coaching. I often try to include word qualifiers in answers, such as words like always, never, etc. if you see these types of words in an answer it is more than likely the wrong answer. In this question I used the words "whole session" to serve as the indicator. Let me know if any of this does not make sense. It is also not the job of a coach to "make sure they can accomplish every goal"-I would argue-although I recognize there is disagreement around this point most likely.
