Medication: 50% Dextrose Injection (D50W)

Precautions/Comments:

-Has a marked sclerosing effect on veins and can be damaging if injected into interstitial tissue. -Monitor IV site for extravasation; aspirate before and during administration. -Ensure patient eats after administration to ensure maintenance of normal blood glucose. -OCEMS has removed D50 from SO and it will be phased out as current supplies are used.

Side Effects:

-Hyperglycemia. -Dehydration.

Classification:

Glucose replacement.

Mechanism of Action:

Increases blood glucose levels to provide immediate source of glucose that can be rapidly used for cellular metabolism.

Contraindications:

Known hyperglycemia.

Pediatric Dose:

-1 mL/kg IVP if > 2 years. -2 mL/kg 25% dextrose if < 2 years. -Do not exceed adult dose. -For pediatric patients < 2 years, dilute to 25% dextrose. Discard one half the volume of the D50W syringe and then draw up the same volume of NS from IV tubing. Rotate to mix prior to administration. -IO access may be used for dextrose administration when patient is unconscious with a blood glucose < 80, unable to establish IV and there is no response to IM glucagon (for IO, use IV dose).

Dosage Form:

-25 gm/50 ml (50%), 50 mL profiled syringe or vial.

Adult Dose:

-50 mL of 50% solution (25 gm) IVP, may repeate once. -IO access may be used for dextrose administration when patient is unconscious with a blood glucose < 80, unable to establish IV and there is no response to IM glucagon (for IO, use IV dose).

Indications:

-A-LOC. -Blood glucose level less than 80 mg/dL.
