The treatments from this chapter are applicable for the following 4 conditions:

• Shock • Cardiac tamponade • Tension Pneumothorax • Hyperbaric oxygen treatment

Management: dehydration

• The first priority is to restore intravascular volume and stabilize the vital signs by replacing fluid deficit with 0.9% saline. • Once the vital signs have been stabilized, maintenance fluid also needs to be administered with D5 0.18% saline. - Step (1) 200mL 0.9% over 10-30min - Step (2) 0-8 hrs. • Deficit: 300 mL of 0.9% saline • Maintenance: 333 mL of D5% 0.18% saline - 9-24 hrs. • Deficit: 500 mL of 0.9% saline • Maintenance: 666 mL of D5% 0.18% saline

Sensitivity of human ear

• The human ear is not equally sensitive to all frequencies • Each curve represents the intensity levels at which sounds of various frequencies have the same loudness • Lowest curve: 0 dB at 1000-Hz has the same loudness as 37 dB at 100-Hz, which means that the ear is more sensitive to a 1000-Hz sound than to a 100-Hz sound • The curve labeled "60" corresponds to sounds that have the same loudness as a 60 dB sound at 1000-Hz • The human ear is most sensitive in the 1- 5 kHz range

Parallel Plates -- voltage and electric field

• The magnitude of the voltage between the plates is proportional to the magnitude of the electric field between the plates • The relationship below is true for the uniform electric field between parallel plates and is true in general for uniform electric fields DeltaV= Ed or E= V/d

Resting membrane potential

• The membrane acts like a capacitor with negative charges on the inner side of the membrane and positive charges on the outside • Resting membrane potential is equal to the electric potential difference across the membrane due to the separation of charges • Typically measures -70 mV • During the resting membrane potential there are: • More sodium ions outside than inside the neuron • More potassium ions inside than outside the neuron

North magnetic pole

• The north magnetic pole is the location at which the magnetic axis crosses the surface in the northern hemisphere • The north magnetic pole is so named because it is the location toward which the north end of a compass needle points • The north magnetic pole does not coincide with the north geographic pole • The north magnetic pole moves over time

Thermography

• Thermography is a technique used for the diagnosis of tumors • Radiation is measured at different points on the human body • Points of high radiation indicate high temperature, which in turn would indicate the presence of a tumor or inflammation • This is like taking a photograph, only in the infrared part of the electromagnetic spectrum. The thermogram shows the temperature variation - the new blood vessels that form around a developing malignancy light up in red, yellow or white, and the normal tissue or benign tumors show up in green or blue. • No radiation and no touch is applied

Indications for Cisternography & Agents

• To evaluate the hydrodynamics of cerebral spinal fluid (CSF), compounds must be instilled into the CSF and their course is mapped • Radionuclide cisternography is one of the most reliable tests to assess CSF Agent: In-111(Indium Pentetate - DTPA)

The non-invasive techniques of measuring ICP are:

• Transcranial Doppler • Tympanic membrane displacement • Optic nerve sheath diameter • CT scan/MRI • Fundoscopy

Active transport

• Transport across living membranes in the direction opposite to that dictated by diffusion probability. • The energy for transport is provided by the membranes, which have a source of energy • Example: active transport is (in part) responsible for Na transfer from the kidney tubules

Diodes

• Unidirectional device (carries current in only one direction) used in rectifier circuits • A diode is a device with two terminals, between which it allows the flow of current in one direction only • Most diodes are made of silicon • One type of diode is the p-n junction diode (n-type + p-type semiconductor)

Ultrasound (sonography)

• Use of high frequency sound waves • Reflected echoes are detected and displayed in real time • Useful in obstetrics, imaging various internal organs including blood vessels (Doppler ultrasound) and heart valves (TTE or TEE) • Painless, fast and easy • Gel is applied to skin, transducer is guided over the skin (pressure my cause mild discomfort, especially if the bladder is full) • Intestinal gas can prevent visualization of deeper structures • Obese patients are difficult to image

Brain Imaging

• Use of nuclear medicine in brain imaging has declined due to the use of conventional imaging modalities (CIM) • Using nuclear medicine has advantage over CIM in that SPECT/PET imaging allows the viewing of deep brain structures and provides a better anatomical view of the brain. • Radiopharmaceuticals used in nuclear medicine provide physiologic information about cerebral pathology which can strongly influence treatment decisions • Used to determine if a patient is legally brain dead (primary clinical use), evaluation of cerebrovascular disease, intracranial injury due to trauma, neuropsychiatric disorders, convulsive disorders and to assess cognitive functions

How does a doppler flow meter work?

• Using Doppler effect, speed of blood flow is measured; increased blood speed indicates a constricted area

How fo vasopressor drugs work?

• Vasopressor drugs make blood vessels smaller and tighter; the body will raise the blood pressure in response to vasoconstriction to keep the blood volume flow rate constant; they are used for short periods of time under close supervision - Increase BP, ex.) phenylephrine, dopamine

Output phase (blood output from ventricle) of cardiac cycle

• Ventricular contraction (systole) • The aortic and pulmonary valves close at the end of the contraction (second sound: dub)

Input phase (blood input into ventricle) of cardiac cycle

• Ventricular expansion (diastole) • The atrioventricular valves close at the end of expansion (first sound: lub)

The invasive techniques of measuring ICP are:

• Ventriculostomy EVD *** gold standard • Microtransducers - Most accurate but introduce risks such as hemorrhaging and infection

• When a small portion of the base layer is negative, the negative white beads are repelled away from it to the upper layer

Sweat varies based on these factors:

• Work rate • Temperature and humidity of the surroundings • Acclimatization state • Baseline hydration level

Ideal properties of a radiopharmaceutical

• easily produced, inexpensive, readily available • has a short effective half-life • particle emission versus photon emission - α- and β-particle emitters are usually therapeutic (μCi), but can be diagnostic (μCi) if able to image associated photons - photon emitters (mostly γ-ray) are diagnostic - EC or IT without IC is most desirable decay properties - γ-ray energies of 30-300 keV are efficiently detected by SPECT cameras. 150 keV is optimal for cameras with a collimator - PET cameras detect 511keV photons

• prepare using a commercial kit • must be checked for particle size by a hemocytometer • > 95% of particles must be: 10 μm < x < 90 μm • < 10 μm will be removed by the liver • > 150 μm will clog arterioles • primarily used for lung perfusion studies

EEG (electroencephalogram)

- Recording of electrical signals produced by the brain • Electrodes are placed at specific locations on the head and electric potential differences that characterize brain behavior are measured and recorded • EEG - plot of electric potential difference verses time • Voltages measured are of the order of 50 microvolts
