Femur and Hip Positioning and X-table Lateral Evaluation Criteria
x-table lateral hip
*cross table or surgical lateral* IR: 10x12 LW grid cassette Position: Supine Elevate patient Flex knee/hip of unaffected side Medially rotate foot 15-20º if applicable CR: Perpendicular to the femoral neck Collimation: 10x12 Respiration: Suspend
Hip Modified Cleaves Evaluation Criteria
No rotation of pelvis Acetabulum, femoral head, and femoral neck Lesser trochanter on the medial side of the femur Femoral neck without superimposition by greater trochanter
x-table lateral hip Evaluation Criteria
Hip joint with acetabulum Femoral neck without overlap from the greater trochanter Small amount of lesser trochanter on the posterior surface of femur Small amount of greater trochanter on the ant/post surfaces of the proximal femur Ischial tuberosity below the femoral head and neck Any orthopedic appliance in its entirety
Hip AP Evaluation Criteria
Hip joint with ilium and pubic bones Proximal one third of the femur Entire long axis of femoral neck not foreshortened Greater trochanter in profile Lesser trochanter usually not projected beyond the medial border of the femur Any orthopedic appliance in its entirety
Hip Modified Cleaves
IR: 10x12 CW or LW Unilateral Hip Position: Center ASIS of affected side Flex knee, draw foot up to the opposite knee Abduct thigh laterally to 45º CR: Perpendicular to the femoral neck Collimation: 10x12 Respiration: Suspend
Hip AP
IR: 10x12 LW Position: Supine Medially rotate the affected leg 15-20º CR: Perpendicular to the femoral neck 2.5" distal on a line drawn perpendicular to the midpoint of a line between the ASIS and the pubic symphysis Alternative: 1-2" medial and 3-4" distal to ASIS Collimation: 10x12 Respiration: Suspend
Femur Lateral
IR: 14x17 LW x2 Position: Patient turns on affected side Proximal Femur: Top of IR at the level of the ASIS Pelvis 10-15º from lateral position Distal Femur: Bottom of IR 2" below knee joint Unaffected limb forward Flex knee 45º, epicondyles ⟂ Pelvis lateral CR:Perpendicular to the mid femur Collimation: 1" on sides, 17" in length
Femur AP
IR: 14x17 LW x2 Proximal Femur: Top of IR at the level of the ASIS Rotate limb 10-15º internally Distal Femur: Bottom of IR 2" below knee joint Epicondyles are parallel to IR CR: Perpendicular to the mid femur Collimation: 1" on sides, 17" in length
Femur Lateral Evaluation Criteria
Most of femur and joint closest to pathologic condition Any orthopedic appliance in its entirety Proximal Femur: Opposite thigh not over proximal femur Greater trochanter SI over distal femoral neck Lesser trochanter visible on medial aspect of proximal femur Distal Femur: Superimposed anterior surface femoral condyles Patella in profile Open patellofemoral space
Femur AP Evaluation Criteria
Most of femur and joint closest to pathologic condition Femoral neck not foreshortened Lesser trochanter not seen beyond medial border of femur No knee rotation Any orthopedic appliance in its entirety
