-immunization status -pertinent history including allergies -results of screenings and exams -individualized health care plan -detailed documentation regarding any visit to the school nurse

-provide direct care to students -provide screening and referrals for health conditions -promote health by providing health education -provide leadership for the provision of health services -promote a healthy school environment
