Emergency communications at the Pentagon site proved challenging on September 11, 2001. Radio communications among emergency responders quickly became overloaded. Arlington County Fire Department Captain Jeff Liebold used foot messengers to inventory onsite units. He sent two firefighters on foot to record the identification number and location of each piece of equipment on the Pentagon grounds because radio communications could not be employed to perform this task. Foot messengers often proved to be the most reliable means of communications during the initial rescue efforts. Fortunately, a surplus of personnel was available at the incident site to serve in that capacity. According to the Arlington County After-Action Report, fire and rescue organizations should establish plans for alternate means of communications during mass-casualty incidents. A number of methods can be employed, including foot messengers.
The Beltway sniper case was an infamous crime, instilling fear in thousands of people. According to the after-action report, communication was clearly the most compelling concern in the sniper case. Investigations of this kind succeed or fail based on executives' ability to effectively manage and communicate information in a timely manner. Incident Commanders must balance the incident needs with the obligations of local executives to be responsive to their citizens. A tornado collapsed a building, trapping 15 people in its basement. Fire department officers immediately designated a fire station located directly across from the incident site as the Incident Command Post (ICP). However, the fire station's confined location and immediate proximity to the incident made it ill-suited for directing the large-scale response effort. As police officers arrived on the scene, they decided to establish their Command Center at a school, several blocks away from the immediate response activities. As response operations progressed and a mobile command vehicle became available, the Incident Command Post (ICP) was established in that vehicle just north of the hospital. Other agencies involved, such as the fire department and emergency medical services, began operating near the new ICP location and Incident Commander. The police department continued to operate from the school. Type a list of potential incident management issues.
The failure to establish a Unified Command structure resulted in miscommunication and confusion. For example, a helicopter requested by the police department arrived at the scene without the Incident Commander's direction or knowledge. Poor communication also made it especially difficult to establish initial security perimeters. Eventually, the police and fire departments swapped radios so they could monitor each other's radio traffic. They also established hourly face-to-face meetings for collaboration and updates. These ad hoc remedies alleviated some of the initial problems. However, miscommunication continued to plague response operations. The after-action report for this incident stated that Incident Commanders need to create Unified Command structures promptly in a multiagency response to an emergency. Establishing Unified Command will help to reduce problems of miscommunication and confusion between response agencies.
Several people were injured as the large truck passed through the crowd and there may be deaths. There is disorder as some attempt to flee and others try to help.
