Decrease in baseline variability progressive tachycardia (>160bpm) decrease in baseline FHR intermittent late decelerations with good variability (HCP needs to be notified)
If ominous patterns occur (Nursing action)
Have mom lay on her left side or place her in knee chest position to alleviate cord compression STOP oxytocin increase IV fluid O2 10-12 L to breathe by mask. apply an internal monitor to verify accuracy of external monitor readings administer amnioinfusion. helps decrease pressure on the cord.
Can you perform a vaginal exam if bleeding is present?
No. Possible placenta previa.
Reassuring FHR patterns
mild variable decelerations less than 30 seconds in duration then return to baseline early decelerations mirror image with contractions accelerations without other charges.
Ominous patterns
persistent late decel no variability, variable decelerations with loss of variability, tachycardia, late return to baseline absence of variability severe bradycardia.
