Rationale for Health Sector Reform

MTPDP

Chapter 12 of the MTPDP 2004-2010 spells out the priority strategies to meet the needs of the poor. The following health priorities were identified: 1. Reduction of the cost of medicines. 2. Expansion of health insurance particularly for indigents through premium subsidy. 3. Strengthening national and local health systems through the implementation of the Health Sector Reform Agenda. 4. Improvement of health care management system. 5. Improvement of health and productivity R and D. 6. Establishment of drug treatment and rehabilitation centers and expansion of existing ones.

Republic Act of 7846

Compulsory immunization for hepatitis for infants and children below 8 years old.

FORMULA ONE for health

Framework for implementation of HSRA. It intends to implement critical interventions as a single package backed by effective management infrastructure and financing arrangements following a sector wide approach. It is directed to ensuring accessible, affordable quality health care especially for the more disadvantage and vulnerable sectors of the populations.

Mission

Guarantee equitable, sustainable and quality health for all Filipino's, especially the poor and shall lead the quest for excellence in health.

The Philippine Health Care Delivery System

In order for the nurse to function effectively, she has to understand the health care delivery system wherein she is working because it influences her status and functions. She needs to properly relate with the dynamics of the political, organizational structure surrounding her position in the health care delivery system.

The reasons why the above conditions are still seen among the population

Inappropriate health delivery system as shown by an inefficient and poorly targeted hospital system, ineffective mechanism for providing public health programs on top of health human resources maldistribution. Inadequate regulatory mechanisms for health services resulting to poor quality health care, high cost of privately provided health services, high cost of drugs and presence of low quality of drugs in the market. Poor health care financing and inefficient sourcing or generation of funds for health care.

Enabler and Capacity Builder

Innovate new strategies in health to improve the effectiveness of health programs, initiate public discussions on health issues and undertaking and disseminate policy research outputs to ensure informed public participation in policy decision making. Exercise oversight functions and monitoring and evaluation of national health plans, programs and policies. Ensure the highest achievable standards of quality health care, health promotion and health protection.

UHC

Is the "provision to every Filipino for the highest possible quality of health care that is accessible, efficient, equitably distributed, adequately funded, fairly financed, and appropriately used by an informed and empowered public". It is a government mandate aiming to ensure that every Filipino shall receive affordable and quality health benefits.

Health Care Delivery System

It is the totality of all policies, infrastructures, facilities, equipment, products, human resources and services that address the health needs, problems and concerns of all people.

Republic Act 1891

It strengthened health and dental services in the rural areas and created rural health units of eight categories of staffing pattern according to population groups of municipalities to be based on a more equitable and scientific distribution of the ratio of personnel to population.

Local

LGU's (Local Government Unit) Provincial and District Hospitals. Health centers/Rural Health Units. Barangay Health Stations.

Private

Largely Market-oriented Health care is paid through user fees at the point of service. Composed of both commercial and business organizations, non-business organizations.

Public

Largely financed through a tax-based budgeting system. Health care is generally given free at the point of service.

Roles and Functions (mandated by Executive Order 102)

Leader in Health Enabler and Capacity Builder Administrator of Specific Services

Administrator of Specific Services

Manage selected national health facilities and hospitals with modern and advance facilities that shall serve as national referral centers and selected health facilities at sub-national levels that are referral centers for local health systems. Administer direct services for emergent health concerns that require new complicated technologies that it deems necessary for public welfare; administer special components of specific programs like tuberculosis, schistosomiasis, HIV-AIDS. Administer health emergency response services, including referral and working system for trauma, injuries and catastrophic events, in cases of epidemic and other widespread public danger, upon the direction of the President and in consultation with concerned LGU. A nurse can have the role of the doctor when given permission by the Secretary of Health in case of epidemic emergency.

Local Heath Board

Mandated to propose annual budgetary allocations for the operation and maintenance of their own health facilities. Function is mainly to serve as advisory body to the local executive and the sangguinian or local legislative council on health related matters.

Letter of Instruction 949 (October 19, 1979)

Minister of Health and all officials and personnel of the Ministry of Health to: 1. Design, develop and implement programs which will: 1.1. Focus on health Development at the community level particularly in rural areas. 1.2. Effectively utilize these systems in order to control or eradicate the immediate and specific health problems confronting Filipino communities. 1.3.Redirect and re-orient the organization of al its agencies at the central level so as to provide efficient and adequate administrative, planning and other support services for health development. 2. Review all projected expenditures within the Ministry's budget including those pro projects funded from external sources, with the end in view of ensuring that the Ministry's financial resources are distributed according to priorities dictated by this Letter of Instructions.

Non-Commercial

Non-profit oriented. Orientation to social development, relief and rehabilitation, community organizing. Socio-civic groups (medical missions). Religious organizations/foundations.

Republic Act 6425

Penalizes the Sale, Administration, Delivery, Distribution and Transportation of Prohibited Drugs. Dangerous Drugs are: 1. "Prohibited drug," which includes opium and its active components and derivatives, such as heroin and morphine; coca leaf and its derivatives, principally cocaine; alpha and beta eucaine, hallucinogenic drugs, such as mescaline, lysergic acid diethylamide (LSD) and other substances producing similar effects; Indian hemp and its derivatives; all preparations made from any of the foregoing and other drugs, whether natural or synthetic, with the physiological effects of a narcotic drug; or 2. "Regulated drug" which includes self-inducing sedatives, such as secobarbital, phenobarbital, pentobarbital, barbital, and amobarbital and any other drug which contains a salt or a derivative, a salt of barbituric acid; any salt, isomer, or salt of an isomer, of amphetamine, such as benedrine or dexedrine, or any drug which produces a physiological action similar to amphetamine, and hypnotic drugs, such as methoqualone or any other compound producing similar physiological effects.

Republic Act 7875

Provides for a universal compulsory health insurance program in the country.

Presidential Decree 856

Provides for control of all factor in man's environment that affect health including the quality of water, food, milk, control of insect, animals, carriers, transmitters of disease sanitary and recreation facilities, noise, unpleasant odors and control of nuisance.

Republic Act 9502

Provides for measures to ensure affordable and quality medicines. Among these measure is the "generics only" requirement. All government health personnel, including nurses, shall prescribe, dispense, transact, and administer using only the generic name of medicines.

Executive Order 503

Provides for the rules and regulations implementing the transfer of personnel, assets, liabilities, and records of national government agencies whose functions are to be devolved to the local government units.

Republic Act 9255

Provides illegitimate children to use the surname of their fathers.

Presidential Decree 825

Provides penalty for improper garbage disposal of garbage and other forms of uncleanliness. All garbage, filth and other waste matters, shall be placed in the proper receptacles for the disposition thereof by garbage collectors.

Republic Act 7600

Provides that babies born in private and government hospitals should be roomed in with their mothers to promote breast feeding.

Republic Act 4226

Requires all hospitals in the Philippines to be licensed before it can offer to serve the community.

Presidential Decree 996

Requires compulsory immunization for all children below 8 years of age against six childhood immunizable disease.

Republic Act 6365

The Congress of the Philippines hereby declares that for the purpose of furthering the national development, increasing the share of each Filipino in the fruits of economic progress and meeting the grave social and economic challenge of a high rate of population growth, a national program of family planning which respects the religious beliefs of the individuals involved shall be undertaken.

Republic Act 9288

The National Newborn Screening System shall ensure that every baby born in the Philippines is offered the opportunity to undergo newborn screening and thus be spared from heritable conditions that can lead to mental retardation and death if undetected and untreated.

Republic Act 8976

The State recognizes that food fortification is vital where there is demonstrated need to increase the intake of an essential nutrient by one or more population groups, as manifested in dietary, biochemical or clinical evidences of deficiency. Food fortification is considered important in the promotion of optimal health and to compensate for the loss of nutrients due to processing and/or storage of food. food fortification, therefore, shall carried out to compensate for the inadequacies in Filipino diet, based on present-day needs as measured using the most recent Recommended Dietary Allowances (RDA).

Republic Act 8749

The State shall pursue a policy of balancing development and environmental protection. To Achieve this end, the frame work for sustainable development shall be pursued. It shall be the policy of the State to: a. Formulate a holistic national program of air pollution management that shall be implemented by the government through proper delegation and effective coordination of functions and activities; b. Encourage cooperation and self-regulation among citizens and industries through the application of market-based instruments; c. Focus primarily on pollution prevention rather than on control and provide for a comprehensive management program for air pollution d. Promote public information and education and to encourage the participation of an informed and active public in air quality planning and monitoring; and e. Formulate and enforce a system of accountability for short and long-term adverse environmental impact of a project, program or activity. This shall include the setting up of a funding or guarantee mechanism for clean-up and environmental rehabilitation and compensation for personal damages.
