Clinical preventive services

1. Adults receiving colorectal cancer screening based on the most recent guidelines 2. Adults with hypertension whose blood pressure is under control 3. Persons with diagnosed diabetes whose A1c value is greater than 9% 4. Children receiving the recommended doses of DTaP, polio, MMR, Hib, HepB, varicella and PVC vaccines by age 19-35 months

Four goals of Healthy People 2020

1. Attain high quality, longer lives free of preventable disease, disability, injury and premature death 2. Achieve health equity, eliminate disparities, and improve the health of all groups 3. Create social and physical environments that promote good health for all 4. Promote quality of life, healthy development, and healthy behaviors across all life stages

Nutrition, physical activity and obesity

1. Increase the proportion of adults who meet the objectives for aerobic physical activity and for muscle-strengthening activity 2. Reduce the proportion of adults who are obese 3. Reduce the proportion of children and adolescents aged 2-19 who are considered obese 4. Increase the contribution of total vegetables to the diets of the population aged 2 and older

Access to health services

1. Increase the proportion of persons with medical insurance 2. Increase the proportion of persons with a usual primary care provider

Reproductive and sexual health

1. Increase the proportion of sexually experienced females aged 15 to 44 who received reproductive health services in the past 12 months 2. Increase the proportion of persons living with HIV who know their serostatus

Tobacco

1. Reduce cigarette smoking by adults 2. Reduce use of cigarettes by adolescents

Injury and violence prevention

1. Reduce fatal injuries 2. Reduce homicides

Environmental quality

1. Reduce the number of days the air quality index exceeds 100 2. Reduce the proportion of children aged 3 to 11 exposed to secondhand smoke

Substance abuse

1. Reduce the proportion of adolescents reporting use of alcohol or any illicit drugs during the past 30 days 2. Reduce the proportion of persons engaging in binge drinking during the past 30 days adults aged 18 and older

Maternal, infant and child health

1. Reduce the rate of all infant deaths 2. Reduce total preterm live births

Mental health

1. Reduce the suicide rate 2. Reduce the proportion of adolescents aged 12 to 17 who experience major depressive episodes

Oral health

Increase the proportion of children, adolescents, and adults who use the oral health care system in the past year

Social determinants

Increase the proportion of students who graduate with a regular diploma 4 years after starting 9th grade
