Criteria for good questionnaire!
Does it provide necessary decision-making information? Does it consider the respondent? Does it meet editing, coding, and data processing requirements?
Flow and Layout
Proper intro, screener questions, general questions first, work questions in middle, sensitive questions last, transitions

Questions to which the repondnet replies in her or his own words..duh. +: creativity -: vague answers, expensive!!!
