Aftercare Code Sequencing (2)

Aftercare codes should be used in conjunction with any other aftercare or diagnosis code(s) to provide better detail on the specifics of an aftercare visit, unless otherwise directed by the classification.

Admission or Encounter for Aftercare Management (1)

Aftercare visit codes (V51-V58) are used when the intial treatment of a disease or injury has been completed but the patient requires continued care during the healing phase or for long-term consequences of the disease.

Admission or Encounter for Aftercare Management (4)

Coding guidelines require that fracture codes be used only while the paitent is receiving active treatment for the fracture and that subsequent encounters be reported with the use of an orthopedic aftercare code. Active treatment is surgical treatment, emergency department encounter and evaluation of treatment by a new physician. Fracture aftercare is cast change or removal, removal of external or internal fixation device, medication adjustment, and f/u visits following fracture treatment.

Admission or Encounter for Aftercare Management (2)

The aftercare code is NOT assigned when treatment is directed toward current acute disease or injury. EXCEPTIONS: dialysis V56.x, chemotherapy V58.11, immunotherapy V58.12, radiotherapy V58.0.

Aftercare Codes Following Surgery

Thease codes should be reported along with any other aftercare codes or other diagnosis codes to provide more detail regarding an aftercare visit.

V-codes Assigned as Additional Diagnosis (1)

To indicate that a patient has a history, health status, or other problem that is not in itself an illness or injruy but may influence patient care. Note: The following V-codes can be listed first if the fact of the history itself is the reason for admission or encounter: V10.x Personal history of malignant neoplasm; V12.4 Personal history of disorders of nervous system and sensor organs; V16-V19 Family history.

V-codes Used as Principal Diagnosis-Situations (3)

To indicate that a person not currently ill is encountering the health service for a specific reason, such as to act as an organ donor, to receive prophylactic care, or to receive counseling.

V-codes Used as Principal Diagnosis-Situations (1)

To indicate that a person with a resolving disease or injury or a chronic condition is being seen for specific aftercare, such as the removal of orthopedic pins.

V-codes Used as Principal Diagnosis-Situations (2)

To indicate that the patient is seen for the sole purpose of special therapy, such as radiotherapy (V58.0).

Two Supplemenatry Classifications in ICD-9-CM V-Codes and E-Codes

V-Codes (V01-V91) are factors influencing health status and contact with health service and E-Codes (E000-E999) which are external causes of injury and poisoning.

V-Codes

V-codes are sesignated as the principal diagnosis in specific situations; others are assigned as additional codes when it is important to indicate a history, status, or problem that may affect health care. Some V codes can be listed as either.

Admission for F/U Exam

A code from category V67 is assigned as the principal diagnosis or reason for encounter when a patient is admitted for the purpose of surveillance after the initial treatment of a disease or injury has been completed.

Admission or Encounter for Aftercare Management (3)

A code from category V57, Care involving the use of rehabilitation procedures, is assigned as the principal diagnosis when the patient is admitted for the sole purpose for rehabilitation following previous illness or injury, with the fourth digit indicating the focus of treatment. An additional code is assigned for the residual conditon requiring rehabilitation. NO code for the original injury or illness that led to the disability is assigned.

Aftercare Code Sequencing (1)

Aftercare codes are generally listed first to explain the reason for the encounter. They can be used occasionally as additional codes when aftercare is provided during an encounter for treatment of an unrelated condition but no applicable diagnosis code is available.

Aftercare Code NOTE!

Do not use Aftercare V-codes for a complication developed from something like surgery. When a patient is admitted because of complications of previous care, the appropriate code from the main classification is assigned rather than the aftercare V code.

Locating E Codes

E codes for poisoning and adverse effects of therapeutic drugs are located in the Table of Drugs and Chemicals in section 2 of volume 2. Other E codes are assgned to indicated the external cause of injuries and poisoning.

E-Codes

E-codes are assigned as additional codes to indicate the responsible medication for any condition described as an advers effect of the correct use of a drug or other medicinal or biological substance. They are also used to report the external cause of an injury and the status of the person at the time the event occurred (civilian or military). They also indicate the activity of the person seeking health care for an injury or health condition that resulted from the activity (heart attack while shoveling).
