AMC Form 33

A form used to identify cargo discrepancies which cannot be corrected on the spot are frustrated. Three copies of the form are required.

AMC Form 106

Log actions and controls used to monitor Biologicals,re-icing, and refrigeration. The log is to be checked at the beginning of a shift by the supervisor.

MLS (Military shipping label) used to identify cargo entering the DTS.

AMC Form 1033

SDDG (Shippers Declaration for Dangerous Goods) verifies all hazards associated with shipment.

DD Form 1502

Series of forms to receive refrigerated cargo. Frozen medical material shipment (Green). When required they must be iced or re-iced with dry ice.

DD Form 1384

TCMD a multipurpose form used to control movement of cargo while in the DTS. Describes all shipping and handling data.
