Gather all needed equipment and supplies. Check expiration dates as needed.
Entry measures 3:
Greet patient and introduce self (Name, ACC nursing student)
Exit measures 2:
4 P's (Pain, potty, position and possessions)
5:
Ask patient if they have any allergies
Exit measures 4:
Both side rails up
Exit measures 5:
Call bell within reach
6:
Explain the procedure to the patient and why this intervention is needed. Answer any questions as needed.

Identifying the patient using two acceptable identifiers (ask patient their name and assess MR#)
Exit measures 3:
Lower bed to the lowest position
Exit measures 6:
Perform hand hygiene as exit room
Entry measures: 1
Perform hand hygiene, put on PPE, if indicated
Entry measures 2:
Provide privacy by closing the door or shutting the curtain
7:
Raise bed to a working height and put down the side rail.
Exit measures 1:
Verbalize to patient that you are leaving
