Exercise Testing for Medical Clearance

-ACSM no longer recommends the inclusion of exercise testing for medical clearance --That decision is left to the qualified health care provider -exercise testing is a poor predictor of acute cardiac events in asymptomatic individuals

The primary physical activity recommendations for Adults

-Adults should move more and sit less throughout the day. Some PA is better than none. -150 min a week to 300 min a week of moderate intensity PA or 75 min to 150 min a week of vigorous intensity

Primary Physical Activity Recommendations

-All healthy adults aged 18-65 yr should participate in moderate intensity aerobic PA for a minimum of 30 min on 5 days a week, or vigorous intensity aerobic activity for a min of 20 min on 3 days a week. -Combos of each can be performed to reach goals -Performing 10 min bouts of moderate intensity for 30 min minimum -Every adult should perform activities that maintain or increase muscular strength and endurance for a minimum of 2 days a week. -Exceeding these is also recommended

Medical history and CV risk factor analysis

-CVD risk factors without underlying disease means substantial risk of adverse exercise-related CV events -Medical and CV screening provides valuable information for designing individualized exercise programs -Medical and CV screening may also uncover a need for other health or medical referrals

Health benefits of Regular PA and Exercise (inversely related to increased risk)

-CVD/CAD -Hypertension, stroke -Osteoperosis -T2DM, metabolic syndrome (Metsyn), Obesity -cancers, -depression -functional health, falls and cognitive function

Algorithm components of ACSM preparticipation screening

-Classifying individuals who do (yes) or do not (no) currently participate in regular exercise -Identifying individuals with known CV, metabolic, or renal diseases or those with --signs or symptoms suggestive of cardic issues -identifying desired exercise intensity

Major signs and symptoms of CVD, metabolic and renal disease

-Pain, discomfort in the chest, neck, jaw, arms, or other areas that may result from myocardial ischemia, or other recent onset pain of unknown origin -Shortness of breath at rest or with mild exertion -Dizziness or syncope -Orthopnea or paroxysmal nocturnal duspnea -Ankle edema -palpitations or tachycarida -intermittent claudication -known heart murmur -unusual fatigue or shortness of breath with usual activities

PA for all

-The ACSM emphasize this -Participation health screening identifies those who are at risk for adverse exercise-related CV events -Individuals should be referred for medical clearance -small but measurable acute risk of activity-associated SCD and AMI -Underlying CVD (cardiovascular disease) who perform unaccustomed vigorous PA are at highest risk --The recommendations are intended to reduce or remove barriers to adopting and maintaining habitual PA or exercise
