What is the first step of measuring height and weight?
Gather needed supplies
What is the 10th step of measuring height and weight?
Raise the height bar above the resident's head.
What is the last the step of measuring height and weight?
Return the scale to the storage area, if required.
What is the 12th step of measuring height and weight?
Slide the large weight bars first and then adjust the smaller weight bar until the weights are balanced again. Add the weights from each bar or activate the display and note the weight.
Perform the beginning five (steps 2 - 6)
knock on the door and wait for permission to enter, introduce yourself and identify the resident, explain what you are planning to do, maintain safety and provide privacy, wash and dry hands thoroughly
Perform the ending six
position the resident, open the room, wash and dry hands thoroughly, report and record, maintain safety place call light within reach, maintain standard precautions no contamination,
what is the 8th step of measuring height and weight?
8) Ask the resident to empty his/her bladder, if needed. Assist as necessary.

9) Assist the resident to remove shoes, or weigh with similar shoes as previous weight.
What is the 13th step of measuring height and weight?
Adjust the height bar to the top of the resident's head so it is level. Read the number where the height rod meets the scale height and note the height. Accurate within .25 inches.

Assist the resident from the scale and to put on his/her shoes
What is the 11th step of measuring height and weight?
Assist the resident to step on the scale with his/her back to the scale, standing upright without holding on to anything. Accurate within 1 pound.
What is the 7th step of measuring height and weight?
Check the accuracy of the scale by moving the weights to the left side of the scale. The scale should balance with the pointer in the center of the scale. If using a digital scale, it should be zeroed before weight is performed.
