What penalty shall be assessed to any team, who, after warning by the Referee, fails to place the correct number of players on the ice and commences play or that causes any delay by making additional substitutions or attempts to the delay the game in any manner. (rule 610 h)

1. There was not enough time for the Referee to stop play for a displaced goal before the shot was taken. 2. The goal must be deliberately displaced by a defending team player or goalkeeper. 3. The puck would have undoubtedly entered the goal had the goal not been displaced.
Under what classifications is body checking prohibited in? (rule 604 a)
12 and under youth age classifications and below, all Girls'/Women's age classifications, and non-check Adult.

If he is in the immediate vicinity of the bench, is completely out of the play, and does so immediately upon breaking the stick (If the player is in the neutral zone, or close to the bench on the same side of the ice and does not have to skate near or around any opposing player, the player can skate to his bench and replace it).
Butt-Ending Criteria (rule 606)
Major plus game misconduct or match penalty for reckless endangerment (attempt to butt-end is included as an infraction).
Any player who recklessly endangers an opponent or causes them to go head first into the boards or goal frame shall be assessed a (rule 608 b)
Major plus game misconduct penalty for Checking from Behind
For a player with a broken stick to successfully replace it and not drop it to the ice, what must he do? (rule 605 Casebook Situation 1 & 3)
Make no effort to participate in play nor be involved in the play (participate means playing or attempting to play the puck, body checking or attempting to body check an opponent, or continuing any action that directly affects the play).
Any player who recklessly endangers an opponent from behind with excessive force while the opponent is in a vulnerable or defenseless position shall be assessed a (rule 608 c)
Match penalty for Checking from Behind.
Elbowing Criteria (rule 611)
Minor (includes attempt to elbow, and then major plus game misconduct or match penalty for reckless endangerment.
Body checking (competitive contact and non check adult classifications) Criteria (rule 604)
Minor and then major plus game misconduct or match penalty for reckless endangerment.
Cross-Checking Criteria (rule 609)

Broken Stick Criteria (rule 605)
Minor penalty for an equipment violation (if the player participates in play with it).
A second violation of the face-off procedure rules by the same team during the same face-off shall result in a (rule 613 d)

Any goalkeeper who shoots the puck directly (non-deflected) outside of the playing area, except when the puck inadvertently leaves the playing area in a location that is not protected by glass or screen shall be assessed a (rule 610 d)

Where does the face-off take place if the attacking team shoots the puck and it goes directly out of play off of the goal post, cross bar, boards, or glass? (rule 612 c)

I am writing "major plus game misconduct if injury" for the penalties that only require a major plus game misconduct to be assessed if that penalty results in an injury. I am writing "(match if necessary)" for the penalties that only require a match penalty to be assessed for attempt to injure or deliberate injury to an opponent. All other penalties that mention a major plus game misconduct or match penalty, but do not have those statements attached with it, have other reasons to assess a major plus game misconduct or match penalty for that particular penalty infraction, than just if it results in an injury or for attempt to injure or deliberate injury. Ex: Checking from Behind.
