Definition: A city employee who is required to hold a CDL and performs safety-sensitive job functions on any city commercial motor vehicle.

Covered Employee.

Part 2 CDL 9A: All covered employees will receive educational materials explaining the DOT's requirements and the City's policies and procedures to meet those requirements. In addition to this policy, the city will provide covered employees with information concerning: 1. The effects of _ and _ on an individuals health, work and personal life. 2. The signs and _ of a drug or alcohol problem. 3. The available methods of _ when a problem does exist.

The second time an employee has a positive test result during the follow-up testing period, the employee shall be terminated.

8 A4: All employees are required to notify the city human resources of any criminal drug statue conviction for a violation occurring in the workplace within _ days after such conviction.

13 C: The appropriate department director or designee shall ensure that an employee required to be tested under this section is tested as soon as practicable and within _ hours of the accident.

13 B1: Pertaining to Covered Employee's: If the alcohol reasonable suspicion test is not administered within _ hours of the incident, the driver shall be placed out of service for _ hours.

8 B Cont.: All safety-sensitive employees returning to duty after a suspension or termination, whether by decree or settlement, as well as any safety-sensitive employee returning to the workplace after any absence of _ days or more, will be required to take a drug test and have a negative result prior to the performance of safety-sensitive job functions.

14 B: Any employee who questions the results of a required drug test may request that a split sample be tested. The split test must be conducted at a second HHS-certified laboratory with no affiliation with the laboratory that analyzed the primary specimen. The employees request for a split sample test must be made to the _ _ _within _ hours of notice of the original sample verified test result.

Appendix A: If the MRO verifies that the officer has a valid prescription for the SAFE TEN drug, the test will be verified as _, but the MRO will discuss with the officer the requirement that the officer needs to inform the department that a prescription medication covered under the SAFE TEN testing program is being used, as per APD Policy 1-04-7.

13 B: Definition: There is objective evidence, based upon known specific, contemporaneous, articulate observations of the employee's appearance, behavior, speech or body odor that would lead a reasonable person to believe that the employee is under the influence of alcohol/drugs; is in possession of or using, transferring, selling or purchasing alcohol or drugs during work hours while on city property or vehicle; Is a public safety employee who has recently illegally possessed, transferred, used or sold a prohibited drug.

7. Definitions: A city position which the director of the human resources department has determined should be classified as safety-sensitive. At a minimum, under part 1 of this policy all public safety employees and employees with access to controlled substances will be classified in Safety-Sensitive Position.

Safety-Sensitive Position.

Appendix A: When a police officer has a positive test result for any one of the SAFE TEN drugs, the MRO will contact the employee and verify the test. A verified positive test result will be delivered to the _ _ _ _, who will notify the Chief of Police.

Appendix A: (Info Card) The SAFE TEN drugs screened: 1. Amphetamines. 2. Barbiturates. 3. Benzodiazepines. 4. Cannabinoids (Marijuana). 5. Cocaine. 6. Methadone. 7. Methaqualone. 8. Opiates. 9. Phencyclidine. 10. Propoxyphene.

When a SAFE TEN test is negative, the Substance Abuse Program manager will initial the results acknowledging receipt and forward the result to the Chief of Police.
