Other reasons: -Does not meet cGMP standards or made in place that does not have cGMP standards -active ingredient is substituted or its strength decreased by mixing with another substance -OTC drug that is not packaged in tamper resistant container -If compounded does not meet PET standards -

A non-resident pharmacy shall be approved by the commission upon a few requirements:

1. Disclose annually in a report to the commission the location, name and titles of all principal corporate officers, if applicable, and all pharmacists who are dispensing drugs or devices to residents of this state. A nonresident pharmacy shall file additional report within 30 days after change in office, corporate officer or pharmacist. 2. Needs to submit a statement that it complies with the directions and requests for information from the licensing agency/regulatory from the state it is located in and comply with all requests made by commission of CT. 3. Maintain at all times a valid unexpired license, permit/registration and be compliant with the law. 4. Also needs to submit a copy of the most recent inspection report from the licensing agency of the state it is located in before receiving a certificate/registration from the department. 5. Must be open at least 6 days/week for a minimum of 40hrs/week 6. Must provide a Toll free number to facilitate communication between patients in this state and the nonresident pharmacy. Has substituted drugs or device for return to care facilities (except as permitted) 10. Has accepted, for return to regular stock, any drug already dispensed in good faith or delivered from a pharmacy, and exposed to possible and uncontrolled contamination or substitution. 11. Has split fees for professional services, including a discount or rebate, with a prescribing practitioner or an administrator or owner of a nursing home, hospital or other healthcare facility 12. Has entered into an agreement with a prescribing practitioner or an administrator or owner of a nursing home, hospital, or other other healthcare facility for the compounding or dispensing of secret formula or coded prescription 13. Has performed or been a party to a fraudulent or deceitful practice or transaction 14. Has presented tot eh commission a diploma, license or certificate illegally or fraudulently obtained , or obtained from a college or school of pharmacy not approved by the commission. 15. Has performed incompetent or negligent work. 16. Falsified a continuing education document submitted to the commission or department or a certificate retained. 17. Has failed to maintain the entire pharmacy premises, its components and contents in a clean, orderly and sanitary condition.
