congestive heart failure- Heart failure does not mean the heart has stopped working. Rather, it means that the heart's pumping power is weaker than normal. With heart failure, blood moves through the heart and body at a slower rate, and pressure in the heart increases. As a result, the heart cannot pump enough oxygen and nutrients to meet the body's needs. The chambers of the heart may respond by stretching to hold more blood to pump through the body or by becoming stiff and thickened. This helps to keep the blood moving, but the heart muscle walls may eventually weaken and become unable to pump as efficiently. As a result, the kidneys may respond by causing the body to retain fluid (water) and salt. If fluid builds up in the arms, legs, ankles, feet, lungs, or other organs, the body becomes congested

First you must prepare the solution with the 4 mL of sterile saline to yield a 4 mL solution containing 5 million units of penicillin. Next you must determine how many milliliters will yield 200,000 Units. To do this, it is best to use dimensional analysis. [200,000 Units] x [4 mL/ 5,000,000 Units] = 0.16 mL You would draw up exactly this amount of medication in a 3 mL syringe. It is not supposed to be rounded up. This would yield 0.2 Units, which would translate into a higher dosage of what was originally intended.

OHA - DRUG CLASS AND MECHANISM: Glipizide is an oral drug that is used for treating patients with type 2 diabetes. It belongs to the sulfonylurea class of drugs which also includes glimepiride (Amaryl), glyburide (Micronase, Diabeta), tolbutamide (Orinase) and tolazamide (Tolinase). Insulin is a hormone that is made in the pancreas that when released into the blood causes cells in the body to remove sugar (glucose) from the blood and reduces the formation of glucose by the liver. Patients with type 2 diabetes have high glucose (sugar) levels in their blood because the cells in their bodies are resistant to the glucose-removing effect of the insulin, and the liver produces too much glucose. In addition, in type 2 diabetes the pancreas is unable to produce the increased amounts of insulin that are necessary to overcome the resistance. Glipizide reduces blood glucose by stimulating the pancreas to produce more insulin.

If the above I.V. has infused 435 cc after four hours, what would be the new rate in gtts/min for the I.V. to finish on time? 36 gtts/min 35 gtts/min 38 gtts/min
Only 435 cc's or mL's have infused after four hours. The question that you need to ask yourself is how many milliliters should have actually infused? Since 1000 cc have to infuse over 8 hours, then after 4 hours, 500 cc's should have infused. However this is not the case. Less than 500 cc's have infused. Now calculate how many milliliters you have left to infuse, which would be 500cc - 435 cc = 65 cc's + 500 cc's = 565 cc's left to infuse over the next 4 hours. Now recalculate the drip rate.[565 cc/ 4hr] x [1hr/60min] x [15gtts/mL] = 35.31 gtts /min or rounding, 35 gtts/min

You are making rounds at 1600 and you notice that one of your patients who is a diabetic has had the following blood sugars @ 0600 B.S. of 68, @1200 B.S. 264. The client is on a insulin sliding scale. The patient appears lethargic, and is difficult to arouse. You notice that the patient is sweating. Which of the following interventions would you begin first? Give the client 6 oz of fruit juice Take the client's blood sugar stat Administer 1mg of glucagon Cover client with 10 Units of Lantus as ordered. 1.5 mL's
Take the client's blood sugar stat- You would only give the client fruit juice if there was no way to take a blood glucose level. Taking the B.S. level is the best plan of action. You would not administer 1mg of glucagon unless prescribed by the doctor based on a very low blood sugar. Glucagon precipitates the release of glucose from the liver by transforming glycogen to glucose. If the client's blood sugar is below normal you would not want to administer the Lantus. Instead you would call the doctor and report the client's blood sugar level. The doctor will probably concur with your judgment to withhold the Lantus at the present time until the client's blood sugar increases back to normal.
