Abd Pain

N/V/D, fever, chills, constipation, hematochezia, hematemesis, dysuria, hematuria

CP

SOB, diaphoresis, palpitations, extremity weakness/numbness, leg pain/swelling, dizziness, cough

Allergic Reaction

difficulty breathing/swallowing, skin rash, tongue swelling, voice change, SOB. Also include if any new meds have been taken

Back Pain

extremity weakness/numbness, saddle anesthesia, bowel/bladder incontinence, fever, chills

laceration

extremity weakness/numbness/paresthesias, other injury, possibility of foreign body, decreased ROM of (body part). Also make sure to include Tetanus shot status

Wound checks

fever, chills, increased warmth/erythema/swelling to the area, drainage from the site. Also include any complaints with their abx

MVC/Fall

head injury/trauma, LOC, back pain, neck pain, hip pain, abd pain, CP, HA, dizziness

SOB

leg pain/swelling, CP, cough

Psychiatric

suicidal ideations, homocidal ideations, hallucinations

If pt if Pregnant

vaginal bleeding/discharge, pelvic pain, abd pain

HA

visual disturbances, extremity weakness/numbness, dizziness, fever, chills, neck pain/stiffness
