1. Form SSA - 721 is completed by the Funeral Director 2. Once you have completed the statement of death by the Funeral Director, the Social Security office will send an application to the family member to apply for the benefit. 3. Lump Sum Death Benefit is $255. 4. Though the Social Security Administration determines who receives the benefit, to be eligible, the following requirements must be met: 5. Funeral Home may not apply for the benefit 6. The family must complete the application within 2 years of the date of death

Application for Presidential Memorial Certificate -This is was certificate signed by the President of the United States recognizing the veteran's military service. -A copy of the DD-214 must accompany the application.

Certified copies are needed for the following: a) To transfer money in bank accounts b) To collect from a life insurance policy c) To change a title/deed, such as to a house or car d) To get into a safety deposit box
