How does a nursing diagnosis differ from a medical diagnosis?

Nursing diagnosis defines patients in response to illness, where as medical diagnosis labels the illness.

Describe the difference between a short-term and long- term goals in the planning process.

Short term goals are those that are achievable within 7 to 10 days or before discharge, where as long term goals take many weeks or months to achieve. Long term goals often relate to rehabilitation.

When should the nursing care plan be constructed?

The nursing care plan should be constructed right after admission database is collected. It must be readily available to each nurse who is assigned to the patient. Once every 24 hours, the care plan is reviewed and updated.

Once the nursing diagnosis have been designated, how are they prioritized?

They are ranked according to their importance. This order can be guided by the hierarchy of needs adapted from Maslow, by the patients beliefs regarding the importance of each problem and by what is most life threatening or problematic for the patient.
