Only when you are handwriting a title of a large work. Underlining means the same thing as italic. But you can not make italics distinct in handwriting so underlining is used instead. Obviously underlining and Italic should never occur in the same essay.

The quotation marks need to go between the final word of the quotation and the first parenthesis. EXAMPLE: "MLA is cool" (Wheeler 28).

This would be done when there are more than one work by the same author being used. If you have more than one author with the same last name, you would include an initial to distinguish them.
