In the plane: - If in VMC: must wear a view limiting device down to the approach minimums. (safety pilot is required) - If in IMC/actual: must remain in IMC until after the final approach fix (safety pilot must be instrument rated) In the sim: - Full approach needs to occur in IMC down to approach minimums.

No. There are two possible outcomes: - If the aircraft will transition from IMC to VMC allowing a landing (in accordance with 91.175) or - If the aircraft will remain in IMC and execute a missed approach at MAP or DA (in both cases, the pilot may log the IAP)
