- the nurse develops a plan of care that prescribes interventions to attain expected and measurable outcomes - INTERVENTIONS NEED TO BE PATIENT SPECIFIC (i.e change dressing every shift, MUST include dressing change materials and steps)

- the nurse evaluates progress toward attainment of goals and outcomes - revisions to care plan can then be made
standard 3 - outcome identification
- the nurse identifies expected outcomes individualized to the healthcare consumer or the situation *(outcomes must be observable)
