X-ray of sella turcica reveals that the cranial vault is intact. Sella turcica was normal in size and shape. There are no intracranial calcifications.

70240

X-ray of the skull in three views shows calvarium to be intact. There is no evidence of any abnormal calcification or increased intracranial pressure. Sella turcica is normal in size and shape. There are a few teeth in the mandible, and there is poor visualization of the lamina dura.

70250

After administration of intravenous contrast dye, soft tissue structures of the neck are identified via computed tomography (CT) scan. The thyroid and parathyroid glands are seen. The right parathyroid gland shows a cyst-like structure. (In addition to the CPT code, assign the HCPCS Level II for supply of the contrast medium.)

Radiologic examination of chest, one view, at 72 inches shows the heart to be transverse in position. Significant enlargement is not apparent on this study. At both bases, there is noted air-bronchogram effect associated with pulmonary infiltration. The thoracic aorta shows atheromatous calcification, and the right acromioclavicular joint shows degenerative arthrosis, as does the thoracic spine, which also shows a scoliosis with convexity toward the right.

71045

AP and oblique x-rays of the right portion of the rib cage show no evidence of fracture or other abnormality of the visualized ribs.

71100-RT

X-ray of cervical spine, two views, was performed; there is no fracture or dislocation identified. Alignment is normal. There is no disc space narrowing. Neural foramina are patent bilaterally. There are no cervical ribs and no soft tissue calcifications in either side of the neck.

72040

CT scan of lumbar spine reveals a right curvature of 3 mm. The vertebra at L4 has a moderate amount of bone spur. Conclusion: Scoliosis.

72131

Left wrist x-ray in frontal and lateral projections reveals an anatomical reduction of the distal radial epiphysis

73100-LT

Left hand x-ray in two views shows some osteoporosis of the visualized bones. There is no evidence of fracture or dislocation. Calcification is seen in the arteries around the wrist.

73120-LT

X-ray, right and left knees, in frontal, oblique, and lateral projections reveals no fracture or dislocation and no joint effusion. IMPRESSION: Negative examination of both knees.

73562-50

One-view x-ray of abdomen reveals no abnormal soft tissue masses, gas shadows, or calcifications. Liver and spleen not enlarged. Visualized bones appear normal.

74018

Serial small bowel x-ray series shows normal progression of barium from the rectum through the small intestine. The jejunum and ileum show a normal caliber and position. There is no evidence of filling defect or obstruction. The mucosal pattern is normal. Impression: Normal small bowel series. (In addition to the CPT code, assign the HCPCS Level II for supply of barium, which is a contrast medium.)

74250, A9698

Following administration of oral contrast medium, gallbladder x-ray indicates satisfactory concentration, the outline is normal, and no calculi are detected. Impression: Normal gallbladder series. (In addition to the CPT code, assign the HCPCS Level II for supply of contrast medium.)

74290, A9698

Drip technique infusion urography reveals that the right renal length is approximately 11.3 cm, and the left is approximately 10.8 cm. Right kidney is higher than the left. Following Renografin, there was minimal delay in excretion from the kidneys and rather poor concentration bilaterally throughout the study, particularly on the right. As far as can be seen, the renal collecting structures are grossly normal. Drip infusion urography shows poorly functioning kidneys. For this reason, one cannot fully assess the kidneys or ureters. As far as can be seen, significant pathology is not apparent. (In addition to the CPT code, assign the HCPCS Level II for supply of Renografin, which is contrast material.)

74410, A9698

A complete view of the heart and cardiac structures via magnetic resonance imaging (MRI) revealed no abnormalities. Heart valves were normal in appearance. Cardiac function was normal. Conclusion: Normal imaging of the heart.
