When should Detemir (Levemir) be administered?

At bedtime because it is a long acting and basal insulin. peaks is 8 to 12 hours after given

Patient is on a sliding scale with Novolog. The patient's blood sugar is 200mg/dl and he should receive 2 units of Novolog according to the sliding scale. The patient is NPO for surgery this am. What should the nurse do?

Hold medication because of surgery because they are NPO. Short acting are held for NPO pts

Can the nurse mix Lantus and Regular insulin together?

No. Only Regular and NPH insulin can be mixed.

NPH (Humulin N)

Intermediate-acting insulin. Onset: 1.5 hours. Peak: 4-12 hours. Duration: 16-24 hours.

Glargine (Lantus)

Long-acting insulin Onset 2 to 4 hours Peak: NO PEAK Duration 24 hours

Detemir (Levemir)

Long-acting insulin Onset is Gradual 1 hour Peak 6-8hr duration: 18-24 hours
