2 kinds of factual information are needed in reporting it

-Objective information -Subjective information

Objective information

-based on what a person sees, hears, touches, or smells -collected by senses -also called signs

NA should listen to what the resident says about

-condition -family -needs

Incontinence

-inability to control the bladder or bowels

odor suggest

-poor bathing -infection -incontinence

look for residents appearance

-rashes -redness -paleness -swelling -discharge -weakness -sunken eyes -posture or gait (walking) changes

To observe accurately, as many senses as possible should be used to gather information

-sight -hearing -touch -smell

Subjective information

-something a person cannot or did not observe -based on something that the resident reported -May or may not be true -also called symptoms

Breathing odor could suggest

-use of alcohol or tobacco -indigestion -poor mouth care
