Right Patient

Right education

patient receives accurate and thorough information about the drug they are taking principle of informed consent educate on possible side effects of the drug answer any questions the patient might have

Right documentation

requires the nurse to immediately record the appropriate information about the drug administered document the patient's response to the medication

Right time

the time the prescribed dose is ordered to be administered health care agencies have policies that specify a range of times which drugs can be given military time is used for documentation

Right Dose

verification that the dose administered is the amount ordered and that it is safe for the patient for whom it is prescribed medication dosage can be based on weight important to assess renal and hepatic function

Components of a complete drug order

1. Patient's name and birth date 2. Date the order is written 3. Provider's signature or name, if an electronic order, T/O or V/O 4. Signature of licensed staff who took the T/O or V/O 5. Drug name and strength 6. Drug frequency or dose 7. Route of administration 8. Duration of administration

Right evaluation

Determines the effectiveness of the drug based on the patient's response to the drug assessing patient for effectiveness of drug, side effects and adverse reactions by ongoing assessment

Right route

Know the correct route of administration by which the drug should be given oral, IM, IV, subq, buccal
