Metabolic changes with burn patients

Pulmonary Burn Injury problems

carbon monoxide poisoning

What happens to a burn pt potassium level at the beginning and once they are healed?

In the beginning they will have hyperkalemia because potassium is moved out of the cell into the blood stream. Once the pt is healed, the patients will possibly have hypokalemia because the potassium moves back into the cell.

IV fluids

Lactated Ringers is fluid of choice -has electrolytes. Good for adults and peds. -Monitor: pain the IV site, allergic reaction and electrolyte overdose

Biologic dressings

Materials obtained from a patient's intact skin (Homograft), cadavers(Allograft), or animals (heter/xenograft) that is used to treat burn victims

fasciotomies

incisions through eschar and fascia. Relieves pressure and allow normal blood flow and breathing

mechanical debridement

Physical removal of debris by irrigation, hydrotherapy or wet-to-dry dressing application Large blisters are left open, but small are covered

Escharotomies

incisions through the eschar down to viable tissue to release the restriction and allow for chest expansion

Hemoconcentration

increase blood osmolarity and hematocrite and hemoglobin r/t vascular dehydration.

Superficial thickness burn

injury to epidermis. mild to severe erythema is noted, and the skin blanches with pressure. Painful Tx: NSAIDS

full thickness burn

Third degree burn. A burn involving destruction of the entire skin; extends into subcutaneous fat, muscle, or bone and often causes severe scarring. Edema is severe.

partial thickness burn

a burn in which the first layer of skin (epidermis) is burned through and the dermis (second layer) is damaged. Burns of this type cause reddening, blistering, and a mottled appearence. (Also called second-degree burn).

caused by contact with chemicals that can burn the skin Treat by flushing burn with lots of cool water to remove chemical, or brush powdered chemical off skin with clean cloth -If digested, effects kidneys and liver
