Qualifications for free burial at Sea

1: Form 21-530-Application for Veterans Burial Allowance, a. Plot interment allowance contained therein. 2: Form 90-2008-Application for United States Flag for Burial Purposes. 3-Form 40-1330-Application for Headstone or Marker, a. Apply to US Army Memorial Affairs Agency or through a VA Cemetery. 4- DD-214-Discharge Papers. 5: DD 1375-Request for Payment of Funeral and/or Interment Expenses.

1: Form SSA-721-Statement of Death by Funeral Director, 2: Form SSA-8-Application for the Lump Sum Death Benefit
Forms Required by the Federal Trade Commission (FTC)
1: General Price List, a. An outdated General Price List must be kept for 1 year after the last use, b. The GPL must be given to the family for retention. 2: Casket Price List. 3: Outer Burial Container Price List. 4: Statement of Funeral Goods and Services Selected.
(VA burial allowance continued) In addition, at least one of the following conditions must be met:
1: The veteran died because of a service-related disability OR 2: the veteran was receiving VA pension or compensation at the time of death OR 3: the veteran was entitled to receive VA pension or compensation but decided not to reduce his/her military retirement or disability pay OR 4: the veteran died in a VA hospital, in a nursing home under VA contract, or while in an approved state nursing home.
You may be eligible for VA burial allowance if:
1: you paid for a veteran's burial or funeral AND 2: you have not been reimbursed by another government agency or some other source, such as the deceased veteran's employer AND 3: The veteran was discharged under conditions other than dishonorable.
How much allotted space per casket is in the selection room?
40-60 square feet per casket.

Application for Military Sea Burial

According to form 21-530:
There is no time limit (to submit the forms) for a service connected burial allowance, plot or interment allowance, VA hospitalization death burial allowance, or reimbursement of transportation expenses.

Burial at Sea must be 3 nautical miles from land
Water must be at least 600 ft deep (in some areas 1800 ft. is required). Reports of all burials at sea must be made to the Environmental Protection Agency. Application for Military Sea Burials are made to the Burial at Sea Coordinator at the desired port of embarkation.
