Battalion chief's / division managers duty

Ensure the proper medical attention is provided to the employee, and that the proper facility is used Notify the appropriate asst chief Notify the communications supervisor if the injury involves emergency transport or death. The communications supervisor will coordinate the appropriate notifications. Review the above forms for completeness and accuracy. Provide a narrative to include details of the incident as well as any corrective action and/or discipline that may be warranted. Ensure that any detailed reports are sent to fire rescue head quarters, safety and wellness bureau, within 24hrs of injury

Supervisor's duty

Notify the next level supervisor and the appropriate captain of safety. If tx. is required phone in notice of injury to the workers compensation carrier at 1-866-867-2643. (24hr #), prior to the end of the shift. Workers compensation will send a copy of the notice of injury to fire rescue headquarters. Complete the authorization for medical tx/drug test form and send with the employee when reporting to the nearest fla hosp centra care location if medical tx is necessary. Make appropriate entries into the station activities log, supervisors log book,met to insure proper documentation.

Off duty injury/illness

Personnel who have suffered an off duty injury or illness are not required to report to florida hosp centra care prior to returning to duty.

Supervisors may consult with who for additional information ?

Employees duty

The employee is required to report all injuries/exposures to their supervisor immediately, no matter how minor they may appear.

If any employee receives a job related injury or illness after hours or injury appears serious

The employee will be taken to the nearest hospital facility for treatment.
