-Identify self and patient name -Explain what is going on wiht the patient that is cause for concern. A concise statement of the problem.
Background
-diagnosis -list of current medications and allergies -most recent vital signs -Lab results (include date/time and results of previous tests for comparison) -Medical history -Recent clinical findings
Assessment
Share the results of clinical assessment -What are the clinician's findings -What is the analysis and consideration of options -Is the problem severe or life threatening
