D-rings and snap-hooks must have a tensile strength of 5,000 pounds

Define: Dee-Ring

True

Employees erecting and dismantling scaffolding are required by osha to provided with fall protection when the distance they can fall exceeds ten feet. true or false?

controlled access zones - attempts to limit employees access. controlled decking zones - positive fall protection is not required, and a plan can be used. safety monitors- warn employees of potential hazards.

Explain how the philosophies of fall protection applied to CA z's, CD z's, and safety monitors

Rip-stitch, tearing or stretching lanyard Rope grab device Retracting lifelines or lanyards

List and define three types of deceleration devices.

Steel lanyards provide no give, thus they have large arresting forces. Nylon rope provides mild arresting forces, however it bounces, so the wearer experiences lots of jolts. (as rule of thumb add 2 feet to necessary pre-fall length of nylon lanyard to accommodate for stretch after the fall) Lanyard stretching/tearing system (known as energy absorber or deceleration device) generally folded and sewn into a small packet on the lanyard, absorbs the shock and prevents bouncing. (reducing the arresting forces)

List and explain the three types of lanyards, and their varying arresting forces.

Erect toeboards, screens or guardrail systems to prevent objects from falling. Erect a canopy structure and keep potential falling objects far enough from the edge of the higher level so that those objects would not go over the edge if they were accidentally displaced. Barricade the area to which objects could fall, and keep employees out of the barricaded area.

List three procedures that employers must implement when workers are exposed to falling objects

False

The height at which fall protection is required depends upon the type of surface/platform the employee is standing

limit the distance of your fall and always use a deceleration device to prevent the sudden jolt.

what is the best way to reduce the arresting forces applied to a workers body?

5,000 for all but self-retracting lifelines which are 3,000

What is rating for D-rings, snap-hooks, lanyards, vertical lifelines, and self-retracting lifelines?

Falls

What is the leading cause of death to construction workers?

a secure point of attachment for lifelines, lanyards or deceleration devices.

define anchorage

a deceleration device containing a drum-wound line which can be slowly extracted from, or retracted onto, the drum under minimal tension during normal employee movement and which, after onset of a fall, automatically locks the drum and arrests the fall.

define self-retracting lanyard

a barrier erected on a roof to warn employees that they are approaching an unprotected roof side or edge and which designates an area in which roofing work may take place without the use of guardrail, body belt, or safety net systems to protect employees in the area.

define warning line system

Lanyards and vertical lifelines must have a minimum breaking strength of 5,000 pounds, while self retracting lanyards must have 3,000 pounds.

define- Lanyard

installed as close as practicable under the walking/working surface on which employees are working, but in no case more than 30 ft. below that level. must be constructed of a strong rope mesh. must be drop tested after initial installation with a 400 lb. bag of sand approximately 30" in diameter, from approximately 42" in height.

describe how safety nets should be installed.

crossbraces can be used as a top rail, if within these measurements, however, a mid rail must still be installed. 38-48" although, not recommended. can be used as midrail 20-30"

explain how scaffolding crossbraces may be utilized as guardrails.

tripping over tools, materials, etc. workers not aware of their location. failure to use required fall protection. dropping objects. lifting people with improper equipment.

list 5 general fall protection hazards.

General fall protection: 6 feet ( subpart M covers the majority of all fall hazards in construction) Scaffolds: 10 feet (subpart L- explained further next) Steel erection: 15 or 30 feet (subpart R-explained further next)

list and explain the three primary sets of OSHA fall protection requirements for construction.

directly to webbing, rope, or wire rope. to other snap hooks. simultaneously to d rings with other snap hooks

list items to which snap-hooks should not be attached.

length of lanyard, deceleration distance, height of worker, safety factor.

list the 4 elements involved in calculating safe fall distance.

Vertical lifelines must be: anchored, independent, protected from damage. Horizontal lifelines must be: Secured to a scaffold, looped around suspension lines about the hoist and brake (on suspension scaffolds), secured between stanchions.

list the basic characteristics of lifelines, include horizontal and vertical

composed of a top rail, midrail, and toeboard. must measure 42" (+or- 3") high to the top rail, with middle rail inserted halfway up. must be able to withstand a force of at least 200 lbs. applied within 2" of the top edge. when guardrails are placed where the hoisting of materials will occur, they must be equipped with a gate.

list the basic characteristics of safe guardrail systems we discussed in class.

must be located directly above the workers in order for them to: avoid swinging, have as short a falling distance as possible, ensure that the drop zone is free of hazards. must be able to withstand 5,000 pounds of force. Don't use guardrails or other items that may break-away. If eye bolts are to be used, they must be rated for loading parallel to the bolt axis. If wall mounted, the rating perpendicular to the axis must be good for 5,000 pounds per employee.

list the basic safe anchorage characteristics we discussed in class. include eye bolts in your description.

thighs, pelvis, waist, chest, shoulders

list the force distribution areas of the body for a full body harness.

Anchorage Body harness Lanyard/lifeline, deceleration device Never use to hoist workers or objects!

list the three components of a personal fall arrest system.

Upper back- used for fall arrest. Side- used for positioning. Front- used for rescue or suspension

list the three types of dee-rings. what is each used for?

stop/prevent the fall - restraint/positioning, guardrails, warning lines, controlled access zones, controlled decking zones, safety monitors. catch the fall - fall arrest, safety nets, catch platforms.

list the two philosophies of fall protection, and the methods for each.

false

shock absorbing lanyards may be hooked to retractable lanyards. true or false?

inspected before every use. keep written daily inspection and training records regarding fall protection. Vital if litigation should arise.

when should fall protection equipment be inspected? what written documents should be kept?
