What is the difference between a Type 1 and Type 2 errors? What is the definition of each?

A Type 1 error is when the null hypothesis is rejected incorrectly and the error is used to determine the level of confidence in the conclusion from the hypothesis test. A Type 2 error is when the null hypothesis is accepted incorrectly. The difference between the two is that a Type 1 error is a false positive result and the Type 2 error is a false negative result.

An RCA is a Root Cause Analysis. An RCA can be used to solve any underlying problems or issues. The procedure for an RCA is to define the issue that needs to be evaluated, gather data that is specific to the issue, apply the 5 Whys for the root cause of the problem, brainstorm corrective actions to change the root causes, and implement the recommendations. An RCA would be appropriate when trying to solve the problem of a patient falling or not getting their medication. The 5 Whys would be useful when determining the root cause.

What is the difference between a RCA and a FMEA? When would you use each?

The difference between a RCA and a FMEA is the RCA happening after the problem or issue has occurred and the FMEA happening before the problem or issue happened. FMEA is the way to anticipate issues before they happen and a RCA is the problem solving after an issue occurred. You would use a RCA when trying to understand why a patient fell. You would use a FMEA when you are testing out all protocols and procedures before an issue happens.
