The Transtheoretical Model of behavior contains a series of six stage used to adequately prepare a person for an eventual change. These six stages are 1) Precontemplation 2) Contemplation 3) Preparation 4) Preparation 5) Maintenance 6) Termination. While many people follow the order set above, many people may travel back and forth between steps in order to change their game plan or adjust their goal. These are steps used to prepare the individual to take action and they create a higher success rate for the individual to reach their goal since they have steps to follow and things to go by.
9.
The Transtheoretical Model or "stages of change model" of behavior describes a series of changes to adequately prepare ourselves for an eventual change. What are these six stages?
Self- Efficacy
The belief that one is capable of achieving certain goals or of influencing events in life. This is one of the greatest influences on health. Individuals who exhibit high _________ are confident that they can succeed and approach challenges positively, which in turn makes them more likely to succeed. On the other hand, someone who has a low self-efficacy may give up easily or never try to change, avoid challenges, and are more likely to revert back to old patterns.

The determinants that can affect an individual's health are; 1) biology and genetics; 2) individual behavior; 3) social factors 4) health services; 5) policymaking and; 6) health disparities.

What are the determinants that can affect an individual's health?
6.
What are the four broad goals for the national Healthy People 2020 initiative?
What is wellness?
__________ is the measure of how healthy a person is over the six dimensions of health. It is the constantly changing process of attempting to achieve a balance as well as the maximum potential of progress in the six dimensions that make a person healthy. To achieve wellness a person must attain the highest level possible in each of the six dimensions, according to their own strengths and limitations.
Health Disparities
are a series of factors that can influence an individual's ability to attain optimal health. These can arise from factors such as race and ethnicity, inadequate health insurance, sex and gender, economics and education, geographic location, sexual orientation as well as disability.
Modifiable Determinants
