Cardiac output is improved because of the increase in contractility and the decrease in left ventricular afterload. The phosphodiesterase inhibitors have a very brief half-life and are occasionally used for the short-term control of acute HF. Phosphodiesterase inhibitors have serious toxicity that limits their use to patients with resistant HF who have not responded to ACE inhibitors, digoxin, or other therapies.
