A student evaluation must include all the following:
- a variety of assessment tools and strategies, including information provided by parents (e.g. tests, homework, parental information) - relevant, functional developmental, and academic information (developmental to see progress as time goes along)
What are the 5 requirements in an individual evaluation?
1. a physical exam (visual and hearing exam) 2. an individual psychological evaluation 3. a social history (what are the child's abilities to function in a community, child development information; and more. This information is provided by the parents) 4. an observation of the student in the current educational placement 5. other appropriate assessments or evaluations (e.g. FBA)
Who can refer a child to the CSE? (4)
1. a student's parent or person in parental relationship 2. a professional staff member of the school district 3. a licensed physician; a judicial officer; the commissioner or designee of a public agency with responsibility for welfare, health or education of children 4. oneself, if the student is 18 or over, or an emancipated minor
What are the four required steps when a referral is submitted by someone other than the parent, student, or judicial officer?
1. state reason for referral 2. include test results, records, or reports upon which the referral is based 3. describe in writing the intervention services, programs or instructional methodologies that were used to remediate the students performance prior to the referral (including supplementary aids, support services) or state reasons why no such attempts were made 4. describe extent of parental contact or involvement prior to the referral
What do we do when we gather all this information from the evaluation?
1. to see if the student actually has a disability or not 2. to write the students IEP - need to know what goals, services, and resources the student needs 3. to know where the student will learn best - can he participate in a general education classroom and what does he need to do that? (always start with seeing if the child can be in a general education classroom)
How many days does the parent have to consent to the evaluation?
30 days from the receipt of referral
What is CSE?
CSE: Committee on Special Education - a multidisciplinary team established in accordance with the provisions of section 4402 of the Education Law

What is an individual evaluation?
any procedures, tests, or assessments used selectively with an individual student, including physical examination, an individual psychological evaluation, a social history and other appropriate and necessary assessments/evaluations to determine whether a student has a disability and the extent of his/her special education needs (excludes basic tests administered to all students in a grade or class)
How often must the student get re-evaluated?
at least once every THREE years
