Definition

The data and information in the health record are clearly defined Guidelines defining H&P content are available to authors and noted in the application user guide.

Precision

The data has expected values. Example: 1965 instead of 65

Timeliness

The data is recorded near or at the time of the visit. Documented at the time of encounter by the authorized provider and available for patient care.

Consistency

The data is reliable For example, required content is the same and available across the encounter and between applications.(The allergy stated in the H&P should be the same throughout the patient stay)

Currency

The data is up to date Information is updated in real-time or within a certain timeframe.

Relevancy

The data is useful. Data obtained by the provider support the plan of care(Positive/negative findings)

Comprehensiveness

The required data is included. Includes all components required by regulatory/accrediting agencies,medical staff rules, and bylaws.
