Comprehensive Interdisciplinary Assessment (CIA) and Plan of Care (POC)
What is the frequency of the CIA/POC for patients new to dialysis or who have changed modality?
An initial CIA must be completed within the first 30 days or 13 treatment. The POC must be developed and implemented within the same 30 day period. A new CIA/POC must then be completed within the 3 months. An annual CIA will be completed 12 months from the 3 month assessment as long as the patient is stable.
What is the frequency of the CIA/POC for patients that transfer in or are transient?
First determine if a CIA/POC was received from the home facility. If NOT received, then an initial CIA/POC must be completed within 30 days. If YES--received, the CIA/POC must be completed within 30 months. If the patient is stable complete the CIA/POC within 12 months of the 3 month assessment and implement the POC within 15 days of the CIA.

What are the criteria for unstable patients?
Extended or frequent hospitilizations, marked deterioriation in health status, significant change in psychosocial needs, concurrent poor nutritional status, unmanaged anemia, and inadequate dialysis are criteria for unstable patients. The interdisciplinary team will reassess unstable areas and updates on the POC and continue monthly re-assessment and/or updates of the POC until the patient is stable or it is determined the condition is chronic and needs no further action required. Must be completed within 3 months. If the patient is stable complete the CIA/POC within 12 months of the 3 month assessment and implement the POC within 15 days of the CIA.
